ART timing in people being investigated for tuberculosis

In the early years of antiretroviral therapy (ART) scale-up in regions where tuberculosis is endemic, clinicians were concerned about co-treatment of HIV and tuberculosis due to shared drug toxicity, tuberculosis-associated immune reconstitution inflammatory syndrome (IRIS), large pill burdens, and drug–drug interactions. Guidelines recommended a cautious approach in patients with tuberculosis symptoms: the priority was first to diagnose tuberculosis, then to start tuberculosis treatment, and to defer ART for several months.

Structural stigma: an urgent intervention priority in Africa

Improved access to antiretroviral therapy (ART) for people with HIV has greatly affected the trajectory of the global HIV pandemic, enabling people with HIV to lead long, healthy lives and curtailing the number of people who newly acquire HIV. Between 2000 and 2024, the number of people with HIV accessing ART globally increased from 370 000 to 31Β·6 million.1 Over the same period, the percentage of people with HIV accessing ART increased from 0% to 83% in Africa,2 a region that accounts for nearly two-thirds of the 40Β·8 million people with HIV worldwide.

Hope for every child: ending paediatric HIV mortality by 2030

Early diagnosis and initiation of antiretroviral therapy (ART) remain the cornerstone for survival of children living with HIV. UNAIDS has set global targets to end paediatric HIV deaths and new infections by 2030. Despite advances in diagnostics, treatment regimens, and access, mortality among children living with HIV remains high. Among people living with HIV enrolled in the global US President's Emergency Plan for AIDS Relief programme, mortality among infants under 1 year and children 1–5 years is 4–9 times and 2–5 times higher, respectively, than that of older age groups.
